Лечението на диабета зависи 80% от болния


"Когато питам пациентите си дали имат роднина с диабет, те ми казват - да, ама той е старчески,
тип II. Смятат го за нещо незначимо, а той се изявява във все по-млада възраст и може да
се прояви дори при деца. Пренебрегването на проблема е свързано с голям риск. При главоломното 
нарастване на страдащите от него е важно всеки човек редовно да се изследва, за да установи 
заплахата навреме, когато лечението е по-лесно и резултатно." 
Това са наблюденията на д-р Наталия Темелкова, ендокринолог в УМБАЛ "Александровска".

Тя съветва хората от рисковите групи, които са с наднормено тегло или имат роднини, страдащи от
диабет, да са по-внимателни към здравето си. А при поява на оплаквания да следят периодично
мастния си профил и въглехидратната обмяна. "Аз бих се смутила от "атрактивната" новина за 
новородено над 4 килограма.
Рядко се обръща внимание на този факт, включително и от колеги, а той е сигнал, че за майката
има риск от диабет", подчертава д-р Темелкова.
Наднорменото тегло е първият знак за метаболитен синдром (МС), който не е равностоен на
диабет, но голяма част от хората с него имат и захарен диабет. Един от елементите на МС е
проблем на въгхехидратната обмяна - нарушена глюкоза на гладно, нарушен въглехидратен
толеранс или изявен диабет. Затова се налага контрол на теглото във всички възрасти, дори
детската.
Според статистиката около 9% от населението у нас над 20 г. има диабет, а около 25% въобще не
са диагностицирани. По данни на СЗО болните се увеличават със 7 млн. всяка година. Стресът и
обездвижването са сред причините да расте главоломно броят на засегнатите и да говорим за
пандемия.
Важно е да се прецени дали един човек има нарушен въглехидратен толеранс или захарна
болест. При диагностициране на диабета някои лекари акцентират повече на стойностите на
кръвната захар на гладно, което в много случаи не е достатъчно. Правят се т.нар. профили - 3
пъти се измерва кръвната захар на гладно - преди закуска, преди обяд и преди вечеря. Нерядко
при диабет тип II, свързан със затлъстяването и с метаболитния синдром, хората имат нормална
кръвна захар на гладно, но при тях патологичните процеси се изявяват първо след ядене. 
Когато се направи един обикновен профил или се мери еднократно кръвна захар, пропускаме
хората, които имат нарушен въглехидратен толеранс, а някои дори имат диабет, но при тях
проблемите са след хранително или въглехидратно натоварване. Има няколко критерия за диабет,
но даже лекарите ги объркват, казва д-р Темелкова. Част от изследванията включват натоварване
с глюкоза, което е единият тест. Той е категоричен по отношение на въглехидратната обмяна, ако
човек е с гранични или с нормални стойности на кръвна захар. Ако е над 11,1 ммол/л и тя се
повтори (за да се изключи лабораторна грешка), налични са и симптоми, тогава не се налага
допълнително уточняване, това е диабет. Ако обаче човек в риск и със симптоми има стойности на
гладно между 6,1-6,9 ммол/л, има смисъл да се натовари с глюкоза. Защото може да има или
нарушен въглехидратен толеранс, или диабет. Възможно е на втория час след натоварването с
глюкоза кръвната му захар да скочи на 12 ммол /л, а да има 6,0 ммол/л на гладно.
Използва се и друг показател за диагностика - гликиран хемоглобин (HbA1C). Той дава
информация за средните стойности на кръвната захар през последните три месеца. Ако са над 6,5
ммол/л, може да се каже, че човекът има диабет. Ако те са между 6,1 и 6,5, трябва да направи
натоварване с глюкоза. Изследването трябва да се направи в сертифицирана лаборатория, иначе
се получава разминаване в резултатите. За съжаление повечето лаборатории у нас не са
сертифицирани, предупреждава лекарката. Според нея в диагностиката има още какво да се
прави, особено при граничните състояния.
Приема се, че при диабет тип II 1,5-2 часа след хране кръвната захар не се променя много, преди
да се променят стойностите на гладно.Така че за точната диагноза невинаги е достатъчно
единичното измерване на захарта, което е най-честият скрининг. Добре е да се има предвид, че
само профил или единично измерване не може абсолютно да изключи наличието на диабет или
предиабет. Ако имаме нарушен въглехидратен толеранс според стойностите през втория час на
натоварването или само нарушена глюкоза на гладно - тези състояния се приемат като предиабет.
И колкото по-рано се диагностицират, толкова по-ефективна е терапията.
Въпреки че разполагаме с всички най-добри лекарства за диабет, това не го прави по-добре
контролиран. В лечението на диабет тип II основната роля е на пациента и тя е над 80%, а не на
лекаря, категорична е д-р Темелкова. Изключително важно е да се спазва определен начин на
хранене, което много затруднява хората. Според нея те са готови да приемат и по 10 вида лекарства,
но да не променят режима на хранене.
И този факт се оказва най-трудната част в котрола на диабета. Освен това пациентите не се
обучават достатъчно, което е най-смисленото и необходимо в борбата с това заболяване. Защото
един непросветен диабетик може да опорочи всякаква терапия, дори и с най-модерните
лекарства, които са ефективни при спазване на определено меню и двигателен режим. От друга
страна, е трудно всеки път диабетикът да прецени какво количество да изяде и каква комбинация
да избере. Затова лекарят дава най-общи препоръки, които трябва да бъдат обсъждани и
повтаряни на всяка среща с пациента. Физическата активност се отразява много добре на
кръвната захар и според натоварването трябва да се съобрази както количеството храна, така и

да си прави гликиран хемоглобин, което не е толкова скъпо. Здравната каса плаща 2 такива
изследвания годишно, но някои пациенти не се възползват дори от тях.
Има и друг парадокс - някои диабетици постоянно си мерят кръвната захар, установяват, че
стойностите и са високи, и нищо не предприемат, за да ги коригират. Контролът на кръвната захар
има смисъл, ако си подготвен и знаеш какво да промениш след това. Ако един скъп медикамент
може да свали стойностите на гликирания хемоглобин с 0,8 до 1,5 %, то едно добро обучение на
пациентите може да постигне същото, и то на неизмеримо по-ниска цена. Има хора, които правят
по 4 инсулинови инжекции дневно, мерят си кръвната захар по 3 пъти, а усложненията настъпват,
предупреждава д-р Темелкова. Усилията си струват само ако резултатът е добър.
Проблемът е, че високата кръвна захар не боли.
При умерено надхвърлени стойности - 10-11 ммол/л, пациентите се чувстват комфортно, нямат
оплаквания. Но всеки процент над нормалните стойности гликиран хемоглобин увеличава в пъти
риска от усложнения. И хората трябва да знаят защо е толкова важно да следят добре кръвната
захар, дори когато нямат оплаквания.
Емоционални натоварвания, различни заболявания, вирусни инфекции, операции винаги се
отразяват на стойностите на кръвната захар. Тя не може постоянно да се мери, но пациентът
трябва да е информиран кога се налага да го прави по-често, тъй като терапията е гъвкава и
изисква постоянно балансиране между хранене, физическа активност, емоции, евентуално друго
заболяване. И диабетикът сам трябва да полага големи усилия за себе си.
Доста са грешките, които се тиражират в свободни разговори между пациенти. Някои смятат, че е
достатъчно да не се яде сладко, което не е вярно. 
Всъщност са разрешени почти всички храни, въпросът е в количеството и баланса колко и какво
да се хапва. Ако един пациент не приема въглехидрати, но набляга на мазнините, крайният
резултат ще е лош. Добре е при диабет тип II да се спазва определена пропорция между
въглехидрати, белтъчини и мазнини при всяко хранене и въглехидратите да са 50%. Има значение
и калоричността на храната, особено при наднормено тегло.
Да се яде много месо, също е заблуда. Рибата, ядките, бобовите храни са добър източник на
протеини. Важно е да се наблегне на бавните въглехидрати, които по-постепенно освобождават
захарта в кръвта, или храните с нисък гликемичен индекс.
Най-сериозната грешка на пациентите е,че може да се хапват плодове без ограничения. А една
малка ябълка има калориите на филийка хляб. Истината е, че не бива да се прекалява както с
месо и мазнини, така и с плодове.
Обявените за диабетични храни не се препоръчват, защото според неотдавнашно изследване
съдържат повече мазнини. С маргарините и подсладителите трябва да се внимава, защото
мненията са противоречиви. По-добре е да се хапне малко масло вместо маргарин.
Спазването на диетичен режим особено от пълен човек с хиперинсулинемия е трудно, защото
тялото му отделя много инсулин и той постоянно е гладен. В повечето случаи пациентът с диабет
тип II отделя повече инсулин в началото на заболяването, но той действа лошо. След храна
инсулинът му започва да действа по-късно и е в повече. Само след 1,5-2 часа той се чувства
гладен и е трудно да преодолее желанието за храна. Ето защо терапията трябва да се прецизира
- не бива да се дава лекарство, което още повече увеличава секрецията на инсулина в ранните
етапи от заболяването. Трябва да се предпише медикамент, който подобрява използването на
инсулина, тогава чувството за постоянен глад отслабва. Налага се популярният при много
диабетици вълчи глад да се контролира с медикаменти. Терапията е индивидуална.
Метформините като тип лекарство са изключително важни - самостоятелно и в комбинация с
други медикаменти, включително и инсулин, защото коригират използването на инсулина в
тъканите.
Би трябвало диабетиците да имат у себе си карта или документ за заболяването, който при инцидент. 
Сега няма такава практика у нас. Много хора не афишират болестта си, защото се страхуват, че 
околните биха се отнесли лошо, а работодателите не биха ги назначили. Това се
отнася особено за младите диабетици, което може да затрудни здравната помощ, ако се наложи.
Тази нагласа в обществото трябва задължително да се преодолее, за да се осигури навременна
помощ на болните при нужда.
Диабетът не е заразен, не е проблем за околните. Въпрос на общи усилия - на здравните
институции, на медиите и на пациентските организации, е да се популяризира информацията за
заболяването и така да се осигури навременна диагностика, ефективно лечение и намаляване на
усложненията му.

  • PIERCE THE SEED

Avocado seeds are typically elongated in shape, but some of them are an exception to this rule. Each seed has a bottom, or the area from whish the root grows, as well as a top, the area from which the plant sprouts. The bottom is more flat, and the top is pointy. It is crucial for you to determine which side is the bottom, and stick it with a toothpick as you put the seed in water in order to avoid submerging it entirely.
  • SOAK THE SEED IN WATER

As mentioned before, place the bottom half of the avocado in water, making sure that the toothpick stands firmly. You can stick the avocado seed at an angle which can ensure that the plant rests in water when set over a glass.
  • LET THE SEED SPROUT

In a period of 3-6 weeks the top part of the seed will split, and a stem sprout will appear. At roughly the same time the roots will grow from the bottom part of the seed.
